Identification

This agreement is between you and MCS. MCS is your NDIS provider. We will help you with supports.
We need to know where you live. This is so we can help you.
We need to know your birthday. This helps us check who you are.
You have an NDIS number. We need it to help with your plan.
This agreement starts on a date. It ends on a date. We tell you both dates.
We will check this agreement every year. We can check it sooner if you want.
Your NDIS plan has a money manager. It can be you, a plan manager, or the NDIA. We need to know which one.
You might have a support coordinator. They help plan your supports. We need their name and number.
Sometimes another person helps you make choices. We write down their name. We write down what they can decide.

About

This agreement is about the supports MCS gives you. It is also about what we both promise to do.
Australian Consumer Law protects you. It applies to this agreement.
Your NDIS supports do not include GST. The law says NDIS supports are GST-free.
This agreement follows the NDIS law. The NDIS law is the Act from 2013.
You can read this agreement in two ways. One way is easy-read. The other way is standard. Both ways are the same agreement.
Your worker may finish a few minutes early. They use this time to write notes with you. They also need time to travel to the next person. MCS pays for this time. It does not come from your NDIS money.
A list shows your supports. It shows what they are. It shows how much they cost.
Tell us what kind of support you want and when. MCS will work out the right NDIS codes and prices for you.
Travel and driving

Sometimes your worker drives you places, or drives to come and help you.

  • Travel costs a small amount for each kilometre.
  • The NDIS sets the most we can charge.
  • You (or the person who helps you decide) choose how much travel you want.
  • We only charge for travel that is fair and that your NDIS plan pays for.
  • We always tell you the cost first.

Responsibilities

Tell us 7 days before you cancel a support. This is the rule.
Tell us straight away if your NDIS plan changes. Tell us if it ends.
Tell us how you want your supports. We listen to you.
Your NDIS plan pays for supports. Help us with the right details so we get paid.
We will give you the supports we agreed. We will do them well.
We follow the NDIS Code of Conduct. It is the rules for good support.
All our workers have a Worker Screening Check. This shows they are safe to support you.
We keep your information safe. We follow the privacy law.
We respect your culture. We respect who you are. We listen to you.
If something goes wrong, we will tell you. We will say what we are doing about it.
If your worker cannot come, we will find another worker. We will tell you first.
MCS has insurance. We can show you the papers if you ask.
We will treat each other kindly. We will be respectful.
We will check how supports are going. We will do this every year.
We will talk about your supports. We will talk about your money.
We will both follow the NDIS rules.

Change

We can change this agreement. We both must say yes. We must write it down.
If we want to change something, we will tell you first. You can tell us what you think.

End

You can end this agreement. Tell us 28 days before you want it to end.
We can end this agreement. We will tell you 14 days before.
If something serious goes wrong, we may not need to give notice. The agreement can end faster.
When the agreement ends, we will sort out any money owed for past supports.

Cancel

Tell us 7 days before you cancel a support. This includes house cleaning and yard work.
If you cancel late, MCS may use your NDIS money to pay for it. This only happens if we still had to pay your worker. It is never money from you.
For allied health like exercise physiology, tell us 2 business days before. If you tell us late, MCS may use your NDIS money to pay for it.

Feedback

Tell us what you think about our supports. You can call, email, or write to us.
You can make a complaint to us. We will listen and try to fix it.
You can also tell the NDIS Commission. They check on providers like MCS. Call 1800 035 544 or visit ndiscommission.gov.au.
You can also call the NDIA about your plan. Call 1800 800 110 or visit ndis.gov.au.
If we disagree about something, we will talk it through. If we still cannot agree, we can ask the NDIS Commission for help.

Sign

Sometimes MCS may share your information with people who help you. You can say yes or no. You can change your mind later.
MCS may take photos at your supports. We use them inside MCS only. You can say yes or no. You can change your mind later.
Sometimes a worker may use your money to buy things for you. You can say yes or no. The worker will give you receipts. You can change your mind later.
I read this agreement. I understand it. I agree to it.
I agree to sign this on a screen. The law says signing on a screen is the same as signing on paper.
I know there are two ways to read this agreement. I know they are the same. Signing one means I agree to both.
